Vaccine Requirement in 2022
Effective January 4, 2022, Kohl Children’s Museum will follow Cook County Department of Health Order 2021-11, which requires proof of COVID-19 vaccination for guests.
Beginning January 4, 2022, the Museum is subject to Cook County Department of Public Health Order 2021-11 which stipulates that guests age 5 and older must show proof of COVID-19 vaccination to enter. No vaccination is required for children age 4 and under.
Proof of Vaccination (age 5+)
Any of the following is acceptable proof of vaccination:
- A CDC COVID-19 Vaccination Record Card
- A digital or physical photo of such a card or record that includes the individual’s name, the vaccine brand administered, and the date the vaccination was administered
- An official immunization record from the jurisdiction, state, or country where the vaccine was administered
- An app that provides a digital record of vaccinations
Proof must be brought with each visit. We are exploring options to record this information if requested by guests.
Required ID (Age 16+)
Guests 16 years of age and older must bring government- or school-issued photo ID that matches the information on the vaccination record. No identification is currently required for guests age 15 and younger per the order.
The order allows for certain medical exemptions. Per the order, individuals who have previously received a medical exemption will be allowed to enter, as long as proof of the medical exemption and a COVID-19 test administered by a medical professional within the last 24 hours is provided.
Guests age 2 and older must still comply with the current State of Illinois mandate requiring face masks regardless of vaccination. Face shields may be substituted for masks for guests with medical exemptions.
We understand that many of our members are frustrated by the mandate or concerned about the prevalence of the Omicron variant. We are working to resolve issues around memberships, which are typically non-refundable, including the ability to pause memberships during the mandate period or during the immediate Omicron surge.
