For Basic and Parties Plus packages only, this form is due 7 days in advance of your party date. This information is required to ensure there’s adequate staff to handle your birthday party.

Please include all adults and children in your total headcount. The only guests who do not need to be included are the birthday child or any infants under 12 mos of age.
